OCC GATE Fall Conference Vendor Application
Thank you for your interest in bringing your business to the OCC GATE Fall Conference! The 2012 Fall Conference will be held at UCI on October 27th. Please fill out the form below, and we will consider your application. Fields marked below with an asterisk (*) are required. Please note that the fee for setting up a vendor table is $100. This fee includes one table, two chairs and one box lunch. Additional tables may be purchased for $100. Vendors may not bring their own tables. Other fees listed below are added in addition to the initial $100 fee.
All applications must be received by October 1st.
Note: All potential vendors MUST sign the UCI Vendor Agreement and MUST fill out the top portion of the UCI Temporary Vendor Permit. According to the UCI Vendor Agreement, vendors must provide a certificate of general liability insurance. To see a sample of a certificate of general liability insurance, please click here.
